If you have moved jobs, or need to update your details held by the HSC Healthcare Library, click here to complete a 'Change of Details' form. This link will take you to the the Library website. Once completed, simply click on 'Submit' at the bottom of the form and it will be forwarded electronically to the Library.

We appreciate you letting the Library know when your details have changed. This ensures smooth communication between you and the Library and that requested material is forwarded to the correct address.
